When there are lots of heavy + medics about, I just use a sniper. A fully charged headshot does enough damage to kill the heavy immediately, or close enough with others beating on them.

As a pyro, I usually go cry in a corner when I see a heavy with his medic hiding five miles away yet still healing the badass.

QFT. Smart medics use me as a meat shield, so at least there is hardly any threats to him from the front. Thus he lives longer, and so do I

I prefer hiding around the nearest corner while still keeping the Heavy in my sight. If there are no corners, I just run around and dodge like crazy(it helps that the medic is the second-fastest class in the game). Hiding behind the Heavy makes you a sitting duck to anyone who manages to flank him.
