I've been having the following problem in portal as well:
On the achievements that involve an ingame counter of some sort (destory grubs, cameras, etc) they worked perfectly the first time through and then the failed counters reset. Well, fair enough, the achievements shouldnt be a case of kill a few grubs the first time you play, kill a few grubs the second, and voila.
BUT
When i now start a new game, the counters just dont count. They are all stuck at 0, meaning i am unable to get a grub squishing/camera destorying/physics punting award.
How do i fix?
