I've experienced the same thing, but only in Garry's Mod (BETA 2007). It only happened once though.

You using a pirated copy of HL2?

It's the source engine's Field of view which is different from most games, you gotta get used to it. Read the source engine wiki for more details, also it only happens when you use 4:3...
