1 On the security camera in Dr. Kleiner's lab the first time you're there.
2Near the beginning when you drop into a cargo container that had a man and a vortigaunt, the vortigaunt was charging the TV and G-Man was on it briefly before it turned off...
3 At the red barn at the start of the airboat chapter, he walks inside, and then disappears, and there are 2 zombies left inside.

After you get a weapon mounted on the airboat, you can see him in a corridor that forks at the end (there is health and headcrabs to the right and the level continues to the left). There are barnacles hanging from walkways (one of which the G-Man occupies).
