Vertex Problem

Ok, I'm somewhat new to 3DSM and I've encountered a problem. I'm still not 100% sure about how objects and their properties work which might be a reason why I can't fix my problem. Well here it goes:
First, I was modeling something with a reference picture in the back.
Then, all of a sudden when I went to create a vertex, instead of creating it where my mouse was, it created it in the middle, not sure if its 100% middle, of my project. The X coordinate was also way off from what I was working with. Let just say if that if I created a polygon around the vertex and connected it to a flat surface from the previous X coordinate I was working on, it would look like a very sharp needle. I've tried to fix this for about and hour now and I've had no luck. At first I thought it might have been some axis constraint settings but then I decided it could be something with the object and its "default" coordinate settings(x,y,z). If you are still confused, I'll try to email you a picture.
 
When creating a new vertex with create polygon, since you're making it in a flat view, it has to put it at the default depth, you're just selecting the x and y position.

You could clone a nearby vertex, and move it over to where it needs to go, then create the polygon connecting it to the other vertices.
 
It's not just the x and y coordinates. When I left click to create a vertex, let's say at the end of the reference picture, it ends up creating it at the center of it vertically but not horizontally. It's like something is constricting my coordinates.
Oh yeah, I know I can continue modeling by just creating a vertex and reinserting the coordinates and moving it around, but it bothers me that somehow I can't just create a vertex wherever I want.
 
It's like I create a vertex at the top right corner but it ends up in the middle(vertically), right. Not only this but the X Coordinates are really messed up(which is not visible in the Left or Right Viewports [The ones I were working on]) making it pop out way more than it should.
